By using Commonwealth Connect, your request is automatically assigned to the responsible department, and you are given a tracking number and contact information in order to be able to follow up on your request if needed. Submitting a report through Commonwealth Connect is quick and easy. Simply enter the address of the issue you'd like to report in the "Street Address" field below and click "Next." On the following page you will be able to choose from a list of report types so that your request can be filtered directly to the appropriate department for the quickest service.
