CHC Application Forms

  1. Click on the appropriate commission link below to download an Application for Certificate form.  (Scroll down on this page to see the list of Commission applications). 

  2. If you need a Demolition Permit Application, please contact the  Inspectional Services Department  and read the information about the  Demolition Delay Ordinance. 

  3. Be sure to look at the  meeting dates and application deadlines page  for important dates. 

  4. Please note that all applications to the Board of Zoning Appeal must be reviewed by CHC staff before submittal to the BZA.  The staff will advise the applicant and the BZA of any jurisdiction that the Historical Commission or one of the neighborhood conservation district commissions may have with regard to a particular project.
