To apply for a Disability Parking space, please submit the following documentation with the Residential Disability Parking Application.  Landlord statement indicating off-street parking is not available.

1.  Physician’s statement indicating need for Disability Parking Space
2.  Photocopy of vehicle registration with disability status
3.  Photocopy of Disability placard along with registration of vehicle
