Fire Alarm Permit Application

This form can be filled out by using your keyboard before printing.

Fire Alarm Permits are issued by the Fire Department. To obtain a Fire Alarm Permit, the completed application and required documents can be submitted in person or by mail to:

If submitting a permit application by mail, please include payment. Permits can be picked up at 491 Broadway. To have the permit returned by mail, please include a self-addressed stamped envelope.

Permit Hours and Fees:

Permit Hours: Mon-Fri 8:00 – 10:00 AM Permit Fee: $50.00

Payment Method:

Cash, Check (make Checks payable to the City of Cambridge)

Required Documentation:

Written impairment letter signed by property owner, agent OR a notarized Fire Protection Impairment Affidavit Copy of Electrician or Fire Alarm Technician License Shop drawings or architectural plans (not required if submitted with building permit plans)

PLEASE NOTE: Related permits are required from the Inspectional Services Department (Electrical Permit) and Electrical Department (Master Box Installation).
