1. Continuation from July 21, 2016. Need update on work schedule; need site visit. Nstar Electric Company, 101 Linwood Street, Somerville, MA 02143, for the following: Binney Street – At Third Street and westerly from MHC3062 a distance of about 695 feet – conduit. This work is necessary to provide service to 17 Cambridge Center (W.O. #2088763).

2. New Petition Nstar Electric Company, 101 Linwood Street, Somerville, MA 02143, for the following: Gray Gardens East – Northwesterly from Pole 768/3 approximately 227 feet west of Raymond Street a distance of about 135½ feet – conduit. This work is necessary to provide service to 11 Gray Gardens East (W.O. #214068).

3. New Petition Nstar Electric Company, 101 Linwood Street, Somerville, MA 02143, for the following: Putnam Avenue – At Green Street and westerly from MHC2178 a distance of about 55 feet – conduit. This work is necessary to provide service to 27 Putnam Avenue (W.O. #2140717).

4. Continuation from July 21, 2016. Comcast must contact other utilities and submit common trench form. Comcast, 426 E. First Street, Boston, MA 02127, for the following: 3-33 Church Street – From existing manhole in front of 50 Church Street, trench easterly 177 feet installing 1-4-inch PVC conduit and one new Comcast manhole in front of 10/28 Church Street.
From newly placed MH, trench southerly 16 feet to 10/28 Church Street installing 1-4-inch PVC conduit and 1 new Comcast sidewalk vault.
From newly placed MH, trench northerly 32 feet to 23 Church Street basement, installing 1-4-inch PVC conduit.
From newly placed MH, trend westerly thence northerly a total of 54 feet to the front of Lizzy’s Ice Cream, installing 1-4-inch PVC conduit and 1 new Comcast sidewalk vault. Continue from new SW vault northerly an additional 16 feet of SW trench to 29 Church Street and 12 feet of SW trench to 27 Church Street installing 1-2-inch PVC conduit for each building.
All 4 sidewalk locations, concrete and brick, will be restored to the same or better condition.

5. New Petition Comcast, 426 E. First Street, Boston, MA 02127, for the following: 7 Channing Street – From existing Pole #10 trench 50 feet westerly in the public way ending at the property line of 7 Channing Street, installing 1-2 inch conduit. This will include the removal and replacement of one sidewalk panel.

6. New Petition Comcast, 426 E. First Street, Boston, MA 02127, for the following: 281 Rindge Avenue – From existing Comcast Manhole, trench northerly 9 feet to the back of sidewalk and install 1-17” X 30” X 12’ vault. From newly placed sidewalk vault extend 1-2 inch conduit to the property line of 281 Huron Avenue. This will include the removal and replacement of one sidewalk panel.

New Petition Massachusetts Institute of Technology, 77 Massachusetts Avenue, Cambridge, MA 02139, for the following: Wadsworth Street – At Wadsworth Street between Main Street and Amherst Street, install 3 new utility poles as shown on T-C500 to temporarily relocate Comcast and Crown Castle utilities to allow clearance for construction activities along the west side of Wadsworth Street, and
At Carleton Street between Deacon Street and Amherst Street, install 2 new utility poles as shown on T-C5020 to temporarily relocate Comcast and Crown Castle utilities to allow clearance for construction activities along the east side of Carleton Street.

8. Continuation from July 21, 2016. Node 12 was previously approved. This is for the conduit to get to the pole. Crown Castle NG East LLC, 1220 Augusta Drive, Suite 600, Houston, TX 77057, for the following: Sidney Street – On Sidney Street, from Pole 7 extending approximately 170 feet southwesterly in four (4) new one and one quarter inch (1¼) conduits to a steel guy pole, proposed Node (N012) on Sidney Street.

9. New Petition Crown Castle NG East LLC, 1220 Augusta Drive, Suite 600, Houston, TX 77057, for the following: Mount Auburn Street – At the intersection of Mount Auburn Street and Banks Street, from an untagged pole extending northwesterly approximately 81 feet in two (2) new 4-inch conduits to Verizon Manhole #26/210 on Mount Auburn Street; and,
On Mount Auburn Street, from Verizon Manhole #26/212 extending easterly approximately 18 feet in two (2) new 4-inch conduits to a proposed vault at the intersection of Mount Auburn Street and DeWolfe Street, intercepting an existing HUIT conduit; and
At the intersection of Bow Street and Arrow Street, from an existing HUIT manhole extending easterly approximately 49 feet in two (2) new 4-inch conduits to existing Verizon Manhole #26/575; and,
On John F. Kennedy Street, from Verizon Manhole #26/373 extending southwesterly approximately 243 feet in four (4) new one and one quarter inch (1¼) and 1 – 4-inch conduits to the intersection of John F. Kennedy Street and South Street continuing on southeasterly approximately 222 feet to Pole #10 on South Street (N044). Also, approximately 11 feet of four (4) new one and one quarter inch (1¼) conduits as a lateral connection to #60 John F. Kennedy Street.
On South Street, installation of telecommunications equipment and pole top antenna on existing wood utility pole #10.
