• Petition continued (1st time) from November 20th P&C hearing subject to obtaining MassDOT permit and adding a 4” conduit for City. Verizon New England, Inc., 125 High Street, Oliver Tower, Floor 5, Boston, MA 02110, for (“Synergetic”) the following: “Monsignor O’Brien Highway”: Install approximately 30 feet of 4-4” conduit from Manhole #195 to Manhole #195-A at 1-5 East St. This work is necessary to clear the way for proposed extension of MBTA Green line tracks on overhead viaduct.

• Petition continued (1st time) from November 20th P&C hearing in order to resolve concerns of abutters. Verizon New England, Inc., 125 High Street, Oliver Tower, Floor 5, Boston, MA 02110, for (“Synergetic”) the following: “Inman Street”: Install approximately 41 feet of 1-4” conduit from Verizon Manhole #26/3 to back of sidewalk and property line between #28 & #30 Inman Street. This work is necessary to place new fiber-optic cable to upgrade service.

• Petition continued (1st time) from November 20th P&C hearing as abutter notifications were flawed. Also voted 3-0 to give Crown Castle permission to approach the other utilities in regard to use of poles and make ready work. Crown Castle NG East LLC, 1220 Augusta Drive, Suite 600, Houston, TX 77057, for the following: an extension of the existing aerial fiber optic network, branching off Pole 13 on Pearl Street and proceeding southeasterly to Sidney Street, then branching both northeasterly and southwesterly. The southwestern branch turns the corner at Allston Street to the untagged pole at the corner of Grove Avenue. The northeast branch on Sidney Street then turns southeast along Erie Street and terminates at Pole 240/2 on Albany Street. New equipment attachments are made at the untagged pole at the corner of Grove and Allston (VZ – N012), at pole 250/2 (VZ – N007) and at pole 8 on Hamilton Street (VZ – N088). And respectfully petitions your commission for permission to erect or construct upon, along, under, or across said way the wires, poles, piers, abutments, or conduits, and other fixtures necessary therefore and located substantially as shown on the plans filed herewith, numbered and dated: N07 (11/7/14), N012 (11/10/14), N088 (11/11/14) and AF015 (undated).

Also an extension of the existing aerial fiber optic network, branching off pole 28 on Pearl Street proceeding northwesterly to pole 7 on Lawrence Street. Also an extension of the existing aerial fiber optic network, branching off pole 550/30 on Pleasant Street proceeding southeasterly terminating at pole 7 on Kelley Road. New equipment attachments are made at pole 7 on Kelley Road (VZ-N019), and at untagged pole on Pacific Street (VZ-N055). And respectfully petitions your commission for permission to erect or construct upon, along, under, or across said way the wires, poles, piers, abutments, or conduits, and other fixtures necessary therefore and located substantially as shown on the plans filed herewith, numbered and dated: N013 (11/11/2014), N019 (11/11/2014), N055 (11/11/2014), N085 (11/11/2014) and CAMAF013 (undated).

The southwestern branch continues to Granite Street then turns southeasterly along Granite Street and terminates at pole 9 on Granite Street. The southeastern branch terminates at the untagged pole at the corner of Hastings Square and Henry Street. Also an extension of the existing aerial fiber optic network, branching off an untagged pole at the corner of Pearl Street and Chestnut Street and proceeding southeasterly along Chestnut Street to Acorn Street, then turning northeasterly along Acorn Street terminating at pole 1. New equipment attachments are made at the pole 9 on Granite Street (VZ-N015), at pole 1 on Acorn Street (VZ-N022), at an untagged pole at the corner of Hastings Square and Henry Street (VZ-N011). And respectfully petitions your commission for permission to erect or construct upon, along, under, or across said way the wires, poles, piers, abutments, or conduits, and other fixtures necessary therefore and located substantially as shown on the plans filed herewith, numbered and dated: N011 (11/10/14), N015 (11/11/14), N022 (11/10/14) and CAMAF014 (undated).

• New Petition Crown Castle NG East LLC, 1220 Augusta Drive, Suite 600, Houston, TX 77057, for the following: permission to apply for licenses from Verizon New England and Nstar Electric to establish fiber optic cable routing and to attach equipment to existing utility poles as outlined in the attached exhibits. It is assumed that if the equipment to be attached to poles is acceptable to and licensed by the utilities, it will also be acceptable to the commission. Crown Castle will subsequently apply for Grants of Location pursuant to Mass. Gen. L. Ch. 166, § § 22-25 (“Massachusetts Pole Attachment Statute”) when route segments are determined to be feasible and acceptable to the City’s Electric and Public Works Departments in accordance with the P&C’s policy of consultation and mitigation of impact to the public.
