1. New Petition Nstar Electric Company, 101 Linwood Street, Somerville, MA 02143, for the following: Dock Street – Southerly from MHC2675 to Deacon Street a distance of approximately 196 feet – conduit.
Deacon Street – Easterly from Dock Street a distance of approximately 292 feet – conduit. Install one (1) new manhole MHC30346.
Amherst Street – At Carlton Street northeasterly from P13/13 a distance of approximately 42 feet – conduit.
Amherst Street – Southeasterly from MHC382 and 100 feet northeast of Carlton street 100 feet a distance of approximately 125 feet – conduit.
Amherst Street – At and southwesterly from Wadsworth Street from MHC3552 a distance of 142 feet – conduit.
Wadsworth Street – At and northeasterly from MHC3532 a distance of approximately 50 feet – conduit. Install one (1) new manhole MHC30347.
This reason for this work is to supply service to The Massachusetts Institute of Technology (W.O. #2137448).

4. Continuation from September 15, 2016, for all licensing to be in place and the make ready work completed. Lightower Fiber Networks, LLC, 80 Central Street, Boxborough, MA 01719, for the following: Brookline Street – Install approximately 4 feet of (1) – 4” PVC conduit and other supporting and protecting equipment from utility pole #34 on Brookline Street southeasterly to the building located at 130 Brookline Street.

5. Continuation from October 20, 2016. The pole is actually on Hovey Street and Lightower needs to notify those abutters. Also need to check and see if there is existing conduit as that street has been recently paved. 6. Continuation from October 20, 2016, to determine if there is any way to get into building. Newly paved street; no available conduit in the area. 7. Continuation from October 20, 2016. Comcast must file common trench paperwork. Comcast, 426 E. First Street, Boston, MA 02127, for the following: Main Street at Portland Street – From Level 3 MH2702 trench 4 feet westerly to proposed 3’X5’ Comcast Vault; continuing 325 feet westerly on Main Street to proposed 3’X5’ Comcast Vault; continuing 362 feet northerly on Portland Street to proposed 3’X5’ Comcast Vault over existing conduits.

Installing (1) 2-inch PVC conduit. This will include the removal and replacement of 1 sidewalk panel. *Common Trench with Verizon petition no. 14.*

9. Continuation from October 20, 2016. Bill Dwyer and Steve Lenkauskas want Verizon to go a different route to minimize excavation in street. Verizon New England, Inc., 125 High Street, Oliver Tower, Floor 5, Boston, MA 02110, for the following: Ash Street – From Verizon Manhole #26/440, place approximately 97 feet of 4-inch PVC to property line at 9 Ash Street. *See petition above.*

10. Continuation from October 20, 2016. Verizon Wireless needs permission to approach utilities. The Commission would like to get an ongoing look regarding small cells on poles. Verizon Wireless, 118 Flanders Road, 3rd Floor, Westborough, MA 01581, for the following: Sheridan Street – Attachment of antenna and equipment to existing utility Pole 630/-2 on Sheridan Street.

11. Continuation from June 16, 2016. Mobilitie has general agreement with Verizon and Eversource; need to reapply for site license. Mobilitie, LLC, 926B Peachtree Street NE, Atlanta, GA 30309, for the following: Palermo Street and Willow Street – Attach equipment to an existing wooden utility pole in the right of way west of the Palermo Street and Willow Street intersection between Windsor Street and Willow Street, near the property line of 14 Palermo Street with electricity connection.
