  7. Accidental Death Benefit - William Slivinsky
    Vote to consider the application for Accidental Death benefits filed by the surviving spouse of William Slivinsky, a retired Police Officer. Mr. Slivinsky died on October 17, 2021. Mrs. Slivinsky is represented by Attorney Thomas Gibson.

  8. Accidental Disability Benefit - Yan Qu
    Vote to consider the reports of the medical panel in the Accidental Disability application filed by Yan Qu, Branch Manager with the Library. Ms. Qu is represented by Attorney Steven Kantrovitz.
