  7. Accidental Death Benefit - William Slivinsky
    Vote to consider the application for Accidental Death benefits filed by the surviving spouse of William Slivinsky, a retired Police Officer. Mr. Slivinsky died on October 17, 2021. Mrs. Slivinsky is represented by Attorney Thomas Gibson.

  8. Accidental Disability Benefit - Yan Qu
